Polymerase Chain Reaction (PCR) is an alternative for RMSF diagnosis in whole blood, but there is less certainty regarding its ability when urine and tissue samples are used. This study aims to estimate the reliability of PCR in skin swab and urine samples obtained from patients with suspected RMSF, comparing it with PCR results from whole blood samples. Samples from 110 suspected RMSF hospitalized patients, between September 2018 and October 2019, were analyzed. Reliability was calculated using the Kappa Coefficient (K). Rickettsia rickettsii was detected in 21 urine samples, finding substantial agreement (K = 0.607, 95% CI (0.385, 0.844)); meanwhile, 3 skin swabs were positive, but without statistical significance (K = 0.417, 95% CI ((-0.338, 1.172).Our findings support that urine PCR is a reliable technique to confirm clinical suspicion of RMSF in hospitalized patients. La determinación de anticuerpos IgM e IgG por inmunofluorescencia indirecta es la prueba estándar para confirmar la fiebre manchada por Rickettsia rickettsii (FMRR), aunque tiene limitado valor clínico pues requiere tres semanas para ser confiable. La Reacción en Cadena de la Polimerasa (PCR) es una opción para el diagnóstico en sangre total, pero hay menos certidumbre respecto a su capacidad al usar muestras de orina y tejido. Nos proponemos estimar la confiabilidad de PCR en muestras de hisopado cutáneo y orina en pacientes sospechosos de FMRR, comparándole con resultados de PCR en muestras de sangre total. Se analizaron muestras de 110 pacientes sospechosos de FMRR atendidos en hospitales de Sonora, entre septiembre de 2018 y octubre de 2019. El cálculo de la confiabilidad se realizó mediante el Coeficiente de Kappa (K). Se detectó Rickettsia rickettsii en 21 muestras de orina, encontrando una concordancia sustancial (K = 0.607, IC 95% (0.385, 0.844)); en tanto, 3 hisopados cutáneos resultaron positivos, pero sin significancia estadística (K = 0.417, IC95% ((-0.338, 1.172). Nuestros hallazgos sustentan que la PCR en orina es una técnica confiable para confirmar la sospecha clínica de FMRR en pacientes hospitalizados.
