Introduction: Using the indicators of the global burden of the disease and the Democratic Development Index of Latin America from 2000 to 2016 their relationship with the state of health in Chile and Mexico was compared. Objective: To characterize the associations to observe their relationship with the state of better or worse health Material and Method: Epidemiological analysis, correlation and coefficients of summary “Z” tests of association were carried out. Results: Chile with high democratic evaluation, presented minimal associations. Mexico with a lower rating presented strong associations between low democratic performance in its indicators such as lower gain in life expectancy, more years lost due to premature death and fewer years of healthy life. Conclusions: A low democratic experience was associated with unfavorable health indicators in Mexico and a better democratic experience was associated with better results in Chile. Introducción: Utilizando los indicadores de la carga global de la enfermedad y el Índice de Desarrollo Democrático de América Latina del 2000 al 2016 se comparó la relación con el nivel de salud en Chile y México. Objetivo: Caracterizar las asociaciones para observar comparativamente, su relación con el estado de mejor o peor salud Material y Método: Se realizó análisis epidemiológico, de correlación y coeficientes de pruebas resumen “Z” de asociación. Resultados: Chile con alta evaluación democrática, presentó mínimas asociaciones. México con menor calificación presentó fuertes asociaciones entre bajo desempeño democrático en sus indicadores como menor ganancia en esperanza de vida, más años perdidos por muerte prematura y menos años de vida saludable. Conclusiones: Una baja experiencia democrática se asoció con indicadores desfavorables en salud en México y una mejor experiencia democrática no tuvo asociación en resultados en Chile.
