It was previously described as a paraneoplastic disease; however, it has been shown that it affects both genders and that the condition may be present without an underlying tumor. We present a case of a 20-year-old female who initially presented headaches, followed by behavioral changes. After that, she progressed to a catatonic state and central hypoventilation, requiring admission to intensive care. During her hospital stay, she presented autonomic dysfunction and dyskinesias. A serum diagnosis of anti-NMDAr encephalitis is made, starting immunosuppressive treatment with steroids, followed by rituximab. The patient had a remarkable improvement and complete recovery from the disease. La encefalitis autoinmune caracterizada por la presencia de anticuerpos IgG en la subunidad GluN1 del receptor N-metil-D-Aspartato (rNMDA) afecta principalmente a mujeres mayores de 18 años y comúnmente se encuentra asociada a un teratoma ovárico, por lo que anteriormente era descrita como una enfermedad paraneoplásica; sin embargo, se ha visto que afecta a ambos sexos y el padecimiento puede estar presente sin un tumor subyacente. Se reporta el caso de una paciente femenina de 20 años que presentó inicialmente un cuadro de cefalea, seguido de cambios conductuales. Posteriormente evolucionó a un estado catatónico e hipoventilación central, por lo que estuvo ingresada en cuidados intensivos. En su estancia hospitalaria presentó disautonomías y movimientos distónicos. Se hace diagnóstico sérico de encefalitis anti-NMDAr, por lo que se inició tratamiento inmunosupresor con esteroides, seguido de rituximab, teniendo una notable mejoría y recuperación completa de la enfermedad.
